My Nuvita Health provides nutrition care through virtual and telehealth services, including but not limited to:
   •    Registered Dietitian consultations
   •    Personalized nutrition counseling
   •    Meal planning support
   •    Educational content and wellness tools
   •    Ongoing care coordination

All Services are dependent on provider availability, insurance coverage, and state licensure regulations.

To use certain Services, you may be required to create a personal account. You agree to:
   •    Provide accurate and updated information
   •    Keep login credentials confidential
   •    Accept responsibility for all activity under your account
   •    Notify us of any unauthorized access immediately

5. Medical Disclaimer

My Nuvita Health provides services from licensed Registered Dietitians. However, we do not offer emergency care or replace your physician, therapist, or mental health provider. Always consult your medical team before making major dietary or health decisions.

If you are experiencing a medical emergency, please call 911 or seek emergency medical assistance immediately.

6. Insurance & Payments

We accept many insurance plans, but coverage is not guaranteed. It is the responsibility of the patient to:
   •    Confirm eligibility and benefits
   •    Understand whether a referral is required
   •    Pay for any portion not covered by insurance (including deductibles, co-pays, or denied claims)
   •    Pay for all out-of-pocket services in full before or at the time of service

7. Refund Policy

Refunds are not issued for services that have already been completed. For prepaid packages or single appointments:
   •    You may request a refund if you cancel within 48 hours of booking and at least 48 hours before your appointment time
   •    Refunds are not guaranteed and are evaluated on a case-by-case basis

8. Cancellation & No-Show Fees

To respect the time of our providers and ensure availability for other patients:
   •    Cancellations must be made at least 24 hours in advance
   •    No-Shows or Late Cancellations will be charged:
   •    $50 for Initial Visits
   •    $25 for Follow-Up Visits
   •    Multiple no-shows or late cancellations may result in the suspension of your account or require pre-payment for future visits

These fees are not covered by insurance.

9. Superbill & Reimbursements

If you are paying privately, we can provide a Superbill upon request. This document can be submitted to your insurance for potential out-of-network reimbursement. Note: Reimbursement is not guaranteed and is at the sole discretion of your insurance provider.

Services may be delivered via secure telehealth platforms compliant with HIPAA. You are responsible for:
   •    Ensuring a reliable internet connection
   •    Accessing care in a private, confidential setting
   •    Downloading or accessing any required technology platforms in advance

Our website may include links to third-party apps, platforms, tools, or services. We do not endorse or control these external resources and are not responsible for their content, functionality, or data practices. Your use of third-party services is subject to their respective terms and privacy policies.

   •    Distribute false, defamatory, or offensive content
